These Documents are Required to Obtain Cells From the UConn Stem Cell Core
1. Completed UConn Stem Cell Core Cell Request Form
2. Documentation of ESCRO Approved Project with the hESC line(s) requested1
3. Documentation of license agreement for hESC line(s) requested2
4. Certificate of training in hESC culture, or explanation of exemption3
5. Purchase order, if applicable4
Notes:
1. The UConn ESCRO will forward the ESCRO approval to the UConn Stem Cell Core upon request. Those from other institutions must obtain a copy of ESCRO approval and forward it to the UConn Stem Cell Core.
2. License agreements vary among hESC-distributing Instutitons. The UConn Stem Cell Core has a "Core" Agreement with WiCell. For WiCell lines, you must fill in two copies of the WiCell SLA, have your Grants Office sign both copies, and turn both copies in to the UConn Stem Cell Core. For institutions other than WiCell, contact the institution directly.
3. If training was received at the UConn Stem Cell Core, a copy of the certificate is not required; just a note on the application. If training was received at another institution, a copy of the certificate should be submitted.
